Fees
Therapy sessions are 55-60 minutes. My fee is $170 for intake sessions and $145 for following sessions. Payment is expected at the time of service. Due to the virtual nature of these sessions, I only accept electronic payment by card. I accept all major credit cards as well as HSA cards. A credit card is required to remain on file and will be charged after each session through your online portal in Simple Practice via Stripe.

Insurance

***Please note I will be private pay only as of 6/22/23. This means I will no longer be taking insurance.***

Until June 22, 2023 I am in network with Premera. My billing company, Prestige Medical Billing, processes these claims. Co-pays are due at the time of service. For insurance clients, your payments are taken by this billing company through Inbox Health via Stripe.

Otherwise, I am an out of network provider. If your insurance offers out of network benefits for therapy, I can provide you with a superbill. This is similar to a receipt for services. You can then submit this to your insurance company and they may provide you with partial reimbursement for the session. Please note I do not accept any Apple Health plans, including Community Health Plan of WA, United Healthcare Community Plan, Molina, Amerigroup, Coordinated Care Corporation.

If you are hoping to use insurance or superbills, I recommend calling the number on the back of your card to ask about your benefits. That way you will know what your co-pay is and if you have a deductible to meet as well as if you have out of network coverage.

Please note that a diagnosis is necessary to bill insurance. We can discuss this together.

Individual Session 30 min. (90832) $75.00 – 16-37 minutes

Individual Session 45 min. (90834) $120.00 – 38-52 minutes

Individual Session 60 min. (90837) $145.00 – 53-60 minutes

Prolonged Services Code (99354) $140.00 – 30-74 additional minutes

No Show/Late cancellation fee- $145 for payment at time of service, Premera allowed rate for insurance clients.

SERVICES NOT COVERED BY INSURANCE

Forms and letters outside of appointment $145.00/hour, billed in increments of 15 min.

Letters for attorneys billed at separate rate $250.00/hour

Electronic Records Fee (email, CD) $6.50/flat rate

Editing of confidential information, per WAC $145.00/hour
